Qualifications and Experience: The successful candidate should possess a High School Diploma and have good comprehension skills, fluency in reading and writing English. Previous janitorial experience especially in a hospital environment is preferred. The post-holder supports safe prescribing by performing clinical medication reviews of admitted or visiting patients (as necessary), ensuring patients receive appropriate medicines prior and post discharge. Set up and manage systems to ensure ‘Best Possible Medication History’ BPMH is recorded for each patient. Qualifications and Experience: The post-holder must possess a Bachelor of Pharmacy or higher (four-year program) and have three (3) years’ working experience as a qualified pharmacist working in a hospital setting. Incumbent must have at least three (3) years’ experience in a clinical capacity, encompassing expertise in ward pharmacy services, drug information and aseptic services. This should be supported by a post graduate clinical pharmacy qualification to diploma/MSc. or equivalent level. Experience in clinical pharmacy training and/or lecturing would be an advantage. Primary Responsibilities: The post holder provides technical support in obtaining, verifying for accuracy, and recording initial medication histories from patients and/or their designated family/caregivers upon presentation to the hospital. This includes conducting patient and/or family interviews focused on obtaining current medication regimens including prescription and over the counter (non-prescription) medications, as well as vitamin and herbal supplements for dosing, frequency, and indications.
